Surgical Treatment of chronic Constipation

Abstract
The most of constipations usually respond well to medical therapy. However, infrequently some
of them are intractable in spite of a long period of medical cares. A few cases we have experien
ced carry life missing of bowel habit often more than ten days over the decade if enema is not
given. These severe type of chronic constipation which is excluded the colonic organic disease or
systemic victim, we would decide to bring the patients into our surgical care with resection of the
affected colon or by-pass surgery. This is a repoty of fice cases who received surgical treatment for
chronic constipation. We will described its results, diagnostic problems and histological findings
obtained from our experiences. Two cases were undertaken subtotal colecromy and cecoproctostomy,
and another two cases were done left side hemicolectomy and remainder was received ileoproctostomy
without resection of the bowel. The all patients were followed postoperatively more than one year
and the results are satisfactory in all cases.
For determination of the surgical indication to chronic constipation, exact and careful examination
are requested. Organic disease of large bowel should be ruled out and psychologic character have
to be analyzed.
In this article, we use the term "Barium Intestinogram". This X-ray examination is the most
practical and valuable method to evaluate the motility of the colon and to conclude the length of
resection of diseased segment of large bowel. Each specimen of resected colon was studied in histo
logically and significant alteration of Myenteric Nerve Plexus was found. The ganglion cell
decreased in number and their nucleus reveals pyknosis, vacuolation and nucleolysis, while the
supporting cell and fibrous tissues rather increased.
From those above findings, we are assure it would be inevitable to make surgical intervention in
intractable chronic constipation.
